When some eps of a given show are already in the library but others are not recognized, if you tell it the name of one of the unrecognized shows and tell it to apply the name to all contents of the library, it creates duplicate entries for all of the previously recognized items as well.
For example, for some reason it only indexed the first 4 (of 6) episodes of season 1 of Primeval. I went to episode 5, manually told it the show, and when it asked whether to apply the name to all episodes in the folder, I said yes. It then created duplicate entries for episodes 1-4.
